AMHC provides Targeted Case Management Services to children ages of 0 to 20 who have a developmental disability or a mental health diagnosis. The focus of AMHC’s Targeted Case Management Service is to include families in the development of their child’s treatment goals and to support the family in developing a sense of strength and empowerment so that they are able to advocate and meet their own needs in the future. Targeted Case Management Services are designed to connect families with resources that will help them meet their goals and make sure the needs of the child are met. Case managers meet regularly with children and families in their homes to develop service plans, identify community resources and advocate for needed supports. They support families by attending school meetings, medical appointments, making referrals, and monitoring services as they progress.
