Community Care Team

The Community Care Team (CCT) is a MaineCare service with no fees for individuals with chronic health conditions helping them to follow doctor’s orders, manage their health, and care for themselves comfortably in their home. The CCT works closely with an individual’s primary care physician. It offers in-home nurse visits, medication education, supports to help patients transition from the hospital to home or rehabilitation; recovery goal setting; help with healthy eating, weight management and smoking cessation; advocacy and support; caregiver support; referral for health, education, and social support services; and referral for mental health and substance use treatment. The service is available in Washington and Aroostook Counties.
