Care Coordination is a person-centered service designed to support individuals in navigating the behavioral health system. At AMHC, our Certified Community Behavioral Health Clinic (CCBHC) Care Coordinators serve as a bridge between individuals and the services they need, ensuring nothing is missed and no one feels alone in their care journey.

 

Whether you're managing mental health concerns, substance use challenges, or co-occurring conditions, Care Coordinators help ensure your treatment is well-organized, efficient, and aligned with your goals.

Our Care Coordinators work directly with you, your family, and your service providers to:

 

  • Develop a personalized care plan tailored to your health goals.
  • Support transitions of care, such as after hospitalization or between providers.
  • Help schedule appointments and coordinate follow-ups to ensure services are delivered and your needs are met.
  • Connect you with community resources, such as housing, transportation, or peer support.
  • Monitor your progress and adapt your care plan as your needs evolve.

 

We also work behind the scenes to ensure services aren’t duplicated and that every provider involved in your care has the information they need to support you.
