Supportive Services: Children’s Behavioral Health Home (CBHH)


When children experience behavioral or emotional challenges, these issues can impact every aspect of their lives, including relationships at school and home, as well as their physical health and overall well-being. At AMHC, the Children’s Behavioral Health Home (CBHH) program adopts a compassionate, team-based approach to support children and families on their journey to wellness.

 

What is CBHH?

CBHH is a comprehensive outpatient program, not a residential facility. It supports children with mental health diagnoses and challenges in adaptive functioning, helping them lead healthier, more fulfilling lives. Funded through MaineCare Section 92, the program brings together a team of professionals, including a health home coordinator, clinical team leader, family support peer specialist, and nurse, who provide care coordination, health education, and direct support to families. Together, this team delivers whole-person care and support.

 

  • Services That Make a Difference: CBHH provides a range of coordinated services to support families in managing their behavioral, physical, and emotional health needs.
  • Case Management: Personalized support to assist families in developing service plans, setting goals, and connecting with community resources.
  • Family Support and Education: Peer specialists provide insights, encouragement, and practical advice based on their own experiences.
  • Care Coordination: The CBHH team collaborates closely with your child’s healthcare providers to align care plans and enhance outcomes.
  • Referrals and Advocacy: CBHH staff are available to support you at every step, whether that involves attending medical appointments, assisting with hospital transitions, or supporting school-related needs such as IEPs, 504 plans, and advocacy within the school system.

This team-based approach ensures that children receive the necessary services and that these services work together to support their development and recovery. Children receiving case management under Section 13 of MaineCare may be eligible to transition to Community-Based Home Health (CBHH), which provides a more coordinated and holistic care experience.

 

Does CBHH Replace Other Services?

By enrolling in CBHH, the behavioral health home team will manage your child’s case. However, services such as therapy, psychiatric care, and other behavioral health supports do not have to change.

 

Getting Started

If your child has MaineCare and struggles with adaptive functioning, they may qualify for CBHH. They do not need to have a prior diagnosis to be considered for services. The CBHH team will complete an in-house mental health assessment and consult with our clinical team to determine a diagnosis. This ensures that appropriate care and access to services are available.
