What Is Care Coordination?

Care Coordination is a personalized service designed to help individuals effectively manage their behavioral health needs. Our Care Coordinators work closely with clients to:

 

  • Develop individualized care plans tailored to their unique goals and needs.
  • Facilitate smooth transitions between different levels of care, such as from hospital to home.
  • Connect clients with appropriate community resources and services.
  • Monitor progress and adjust care plans as necessary to ensure optimal outcomes.

 

This approach ensures that each person receives the right care at the right time, reducing the risk of gaps in treatment and promoting long-term recovery.

 

Who Can Benefit?

Our Care Coordination services are available to individuals across Aroostook, Hancock, and Washington counties, regardless of insurance status or ability to pay. We particularly focus on supporting:

 

  • Individuals with co-occurring mental health and substance use disorders.
  • Those transitioning between care settings who need assistance navigating the healthcare system.
  • Clients who may not qualify for traditional case management services but still require coordinated support.

 

AMHC’s Comprehensive Approach

Recent Enhancements

In 2024, AMHC was awarded a $4 million Certified Community Behavioral Health Clinic (CCBHC) Improvement and Advancement Grant from the Substance Abuse and Mental Health Services Administration (SAMHSA). This funding allows us to:

 

  • Expand Care Coordination services, including specialized support for veterans.
  • Enhance access to crisis support, screening, diagnosis, and outpatient care.
  • Strengthen partnerships with community organizations to provide comprehensive, wraparound services.
